Huge paraduodenal hernia

Congenital internal hernia is one of the rare causes of small bowel obstruction. Paraduodenal hernia is the most common variety of congenital internal hernia followed by Transmesenteric and Transomentalvarieties. we operated a 50 yrs old gentleman for intestinal obstruction. Preoperative CT scan shown the left paraduodenal hernia. diagnostic laparoscopy revealed a huge Hernia Sac in the left paradudenal region. Laparotomy was done to see jejunum and proximal ileum in the sac with narrow mouth but there was no gangerene in the bowel wall. We reduced the bowel after widening the mouth, the sac excised and the theare of the sac made in continuity with rest of the peritoneal cavity. Postoperative recovery was uneventful.
